Know Your Options

Medical treatment decisions are a matter of personal choice. You have the right to know about your medical condition, the treatment alternatives open to you, the various risks and benefits involved, and the steps you can take under State law to ensure that your wishes are known and followed.
 

Payment of home care services:

• Self-pay by the individual receiving care

• Public third-party payor including Medicare, Medicaid, and Social Services Block Grant

• Private third-party payor including commercial health insurance, Worker’s Compensation, Champus and managed care organizations.

Are you homebound? __Yes __No (physical condition and/or limitations are such that it would be considerable and taxing effort for the patient to leave home. Absences should be infrequent and of short duration.)
 

Do you have a skill or medical need requiring assistance or a nurse or therapist? __ Yes __ No
 

Have you been placed on a new medication or has the dosage of one of your current medication been changed recently? __ Yes __ No
 

Are you experiencing a current change or problem with a chronic disease process? __ Yes __ No
 

Is there a procedure, disease process or medication instruction that needs to be taught to you or your caregiver? __Yes __ No
